Asians Should Be Treated When HBV DNA and ALT Levels Are Low to Moderate

Hong Kong researchers have called for lowering the viral load (amount of HBV
DNA in the blood) and ALT thresholds for treating HBV-infected Asian
patients. Researchers urged doctors to treat patients with antivirals to
lower viral load, even if HBV DNA levels and ALT levels are not high, given
the damage the infection does over decades to people who have been infected
since chidhood. Researchers made the recommendation in a study published in
the August 2007 issue of the Journal of Gastroenterology and Hepatology. “In
patients who acquire the disease in early life … complications continue to
develop because of the prolonged insidious damage to the liver,” they wrote,
even when HBV DNA levels are low to moderate. Developing “e” antibodies and
lowering viral load to just below 100,000 copies/ mL may not be adequate,
they wrote. “More recent studies have shown that in order to move towards a
better disease outcome, patients should have HBV DNA levels at least less
than 1,000 copies/mL, with ALT levels preferably in the range of less than
0.5 times the upper limit of the normal range,” they wrote. “Therefore,
prolonged, adequate suppression of viral replication should be the practical
goal for the treatment of chronic hepatitis B disease in the Asian
population.” Current guidelines recommend withholding treatment until ALT
levels are twice normal and HBV DNA levels exceed 100,000 copies/mL.

也是中国香港特区做的研究，查查以前香港黎青龙教授说的，我觉得有道理，只要HBV-DNA阳性都要考虑治疗，这点和国内缪晓辉教授说的很像．
