PREDICTORS OF SUSTAINED VIROLOGIC RESPONSE TO
PEGYLATED INTERFERON THERAPY IN CHRONIC HEPATITIS B
: A REAL-LIFE STUDY

Background: Hepatitis B virus (HBV) infection remains a
major public health problem. Information on the efficacy of
pegylated interferon (PEG-IFN) treatment of chronic hepatitis
B (CHB) patients and predictors of the sustained virological
response based on real-life data is limited The study was
aimed to evaluate virological and histological outcomes and
predictor response after a complete therapy of Peg-IFN.
Methods: Treatment naïve CHB patients were enrolled in this
prospective study during 2016-2019 Patients received PEGIFN
α2a 180 μg once weekly for 24 or 48 weeks for HBeAg-positive
and HBeAg-negative CHB Virological response is
defined as serum HBV DNA levels <2,000 IU/ml at 6 months
and at the end of therapy Sustained off-therapy virological
response is defined as serum HBV DNA levels <2,000 IU/ml
for at least 12 months after the end of treatment Results:
This study involved 96 patients, 65% were male, mean age
was 36 6 ± 15 5 years, 47 9% HBeAg positive, and 14 6%
with cirrhosis Virological response (VR) was observed in
72,9% patients (70/96), Sustained off-therapy virological
response (SVR) was achieved in 23,2% patients (23/96), and
decreasing value of fibrosis was observed in 73.2% patients
after 1 year Age <35 years, HBeAg positive, and Viral
load<log 6 IU/ml were significantly associated with virological
response (VR) at the end of therapy (p 0 04, p 0 030, and
p<0 001) The week 96 multivariate logistic regression
analysis showed that in patients with baseline HBV DNA < log
6 IU/mL, ALT >2x upper normal limit, and age <35 year were
more likely to achieve SVR (OR 12 1, 95%CI : 3 99 – 37 09
p<0 000; OR 3 1, 95%CI : 1 10-8 74 p<0 032; OR 5 31 95%CI
: 1 691-16 68 p<0 004; respectively) Conclusion: Peg-INF
therapy in the management of CHB showed good virological
and histological response In this study, young adult, ALT
elevation, and low viral load were predictors for sustained off virological response.
