Serum Hbcrag Is Better Than HBV RNA and
HBsAg in Reflecting Intrahepatic Covalently
Closed Circular DNA

Background: This study aimed to investigate the correlation
of serum hepatitis B core-related antigen (HBcrAg) with serum
HBV RNA and HBsAg, and further reveal whether serum HBcrAg
is the best surrogate marker for intrahepatic HBV covalently
closed circular DNA (cccDNA) when compared with serum
HBV RNA and HBsAg in HBeAg-positive patients. Methods:
Serum HBcrAg, HBV RNA, HBsAg and intrahepatic cccDNA
were quantitatively detected in 85 HBeAg-positive patients
who have never received antiviral therapy. And correlations
between serum HBcrAg, HBV RNA, HBsAg, and intrahepatic
cccDNA were evaluated using multivariable linear regression
and Pearson’s correlation analysis. Results: HBcrAg was
correlated positively with HBsAg (r=0.564, P<0.001) and HBV
RNA (r=0.445, P<0.001), and HBV RNA was also correlated
positively with HBsAg (r=0.323, P=0.003). By multivariable
linear regression, HBcrAg (b=-0.57, P<0.001), HBsAg (b=-
0.324, P<0.001), and HBV RNA (b =0.177, P=0.003) were
all associated with intrahepatic cccDNA levels. And HBcrAg
was better correlated with intrahepatic cccDNA (r=0.843,
P<0.001) as compared to HBsAg (r=0.710, P<0.001) and
HBV RNA(r=0.541,P<0.001)(Figure). Though HBcrAg levels
were influenced by hepatic inflammatory activity (10.26±1.40
log10 U/mL for <G2 vs. 8.86±1.63 for ≥G2, P<0.001 ) and HBV
DNA levels (9.46±1.57 log10 IU/mL for <8 log10 IU/mL vs.
10.44±1.43 ³8 log10 IU/mL,P=0.004), the good correlations of
HBcrAg with intrahepatic cccDNA still existed after stratified
by inflammatory activity (r=0.852 for <G2 and r=0.743 for
≥G2) and HBV DNA levels (r=0.848 for<8 log10 IU/mL and
r=0.828 for ≥8 log10 IU/mL). Conclusion: There is a positive
correlation between HBcrAg, HBV RNA and HBsAg in HBeAgpositive
patients, but HBcrAg is better than HBV RNA and
HBsAg in correlation with intrahepatic cccDNA.
