国内获批的首个PD-1抑制剂于近日上市

我国迈向免疫肿瘤治疗时代 肺癌晚期患者有了新选择

　　浙江在线9月4日讯（浙江在线记者 李文芳 通讯员 李会芳）继今年6月15日获国家药品监督管理局批准后，我国首个获批的免疫肿瘤（I-O）治疗药物，也是目前唯一获批用于肺癌治疗的PD-1抑制剂---纳武利尤单抗注射液于8月28日正式在国内上市。

　　我们迎来了中国免疫肿瘤治疗时代。

　　中国抗癌协会常务理事、浙江省抗癌协会理事长、浙江省肿瘤医院主任医师毛伟敏教授于近日在媒体沟通会上表示，目前在非小细胞肺癌免疫治疗研究中，纳武利尤单抗是随访时间最长的PD-1抑制剂，晚期患者五年生存率由过去的不到5%提高到了16%，已成为全球大多数国家和地区二线非小细胞肺癌的标准治疗。

  “作为中国首个、也是目前唯一获批用于肺癌治疗的PD-1抑制剂，纳武利尤单抗的上市开创了国内肿瘤治疗的新时代。”

　　免疫肿瘤治疗为晚期肺癌患者带来新希望

　　肺癌是我国最为高发的疾病之一，其发病率和死亡率居所有癌症之首。

　　在我国，每年肺癌新发病例达78.1万例，相当于每10分钟就有15人罹患肺癌。由于肺癌早期症状较为隐匿，68%的肺癌患者在确诊时已为晚期，而晚期患者过往的五年生存率不超过5%。

　　目前，我国肺癌的主要治疗手段包括手术、放疗、化疗和靶向治疗。尽管治疗手段不断推陈出新，但对晚期患者，特别是鳞癌及无驱动基因的患者而言，当下国内的治疗手段相对有限，无法满足患者亟需改善治疗现状、延长生存期的需求。

　　浙江大学医学院附属第一医院呼吸内科参与了纳武利尤单抗在中国的注册临床研究（CheckMate-078），呼吸内科主任周建英教授告诉记者，这是第一个在中国启动的PD-1抑制剂的临床研究，首次证实了PD-1抑制剂在中国人群中的有效性和安全性均优于标准化疗，患者生存获益明显，死亡风险可降低32%。“这与之前的国际大型临床研究结果一致，东西方人群无显著差异。无论患者PD-L1表达与否，鳞癌和非鳞癌患者都能达到临床获益。”周建英教授说。

　　纳武利尤单抗在国内获批的适应症为，用于治疗表皮生长因子受体（EGFR）基因突变阴性和间变性淋巴瘤激酶（ALK）阴性、既往接受过含铂方案化疗后疾病进展或不可耐受的局部晚期或转移性非小细胞肺癌（NSCLC）成人患者。

　　激活患者自身免疫系统抗击肿瘤

　　免疫肿瘤治疗，跟传统直接攻击癌细胞的方式不同，它是通过激活人体自身免疫系统来抗击肿瘤，已成为国际上肿瘤治疗的主要手段之一。

　　毛伟敏教授介绍说，免疫肿瘤治疗包括免疫检查点抑制剂和共刺激激动剂。而免疫检查点抑制剂是目前国际上肿瘤免疫治疗的主要研究方向，PD-1/PD-L1抑制剂、CTLA-4抑制剂均属于此类。

　　以免疫检查点抑制剂为例，正常情况下，人体内的免疫T细胞可以监测并清除肿瘤细胞。然而肿瘤细胞非常狡猾，它能开拓“监管”通路，当肿瘤细胞表面的PD-L1/PD-L2与免疫T细胞结合后，T细胞将减少增殖或失活，此时肿瘤细胞将有机会躲避免疫系统的攻击。

　　免疫检查点抑制剂正是通过抑制T细胞表面的PD-1与肿瘤细胞表面的PD-L1/PD-L2结合，重新激活T细胞的肿瘤识别功能，并将其消灭。其核心是重新激活肿瘤患者T细胞的抗肿瘤 反应。纳武利尤单抗，就是一种PD-1免疫检查点抑制剂，正是通过激活人体自身免疫系统抗击肿瘤，从而达到使患者长期生存获益的目的，具有毒副作用小、疗效持久等特点。

　　目前，纳武利尤单抗已在全球超过65个国家和地区获得批准，包括美国、欧盟、日本和中国。在全球，纳武利尤单抗已获批17项适应症，涉及肺癌、黑色素瘤、肾癌、霍奇金淋巴瘤、头颈鳞癌、膀胱癌、结直肠癌、肝癌、胃癌等9个瘤种。

　　降低治疗费用延长生存周期

　　目前纳武利尤单抗的建议零售价格分别为：100mg/10ml 9260元（大瓶）、40mg/4ml 4591元（小瓶）。相比此前媒体报道的德国代购大瓶纳武利尤单抗（13500-15500元），国内上市价格还要便宜不少，比香港直邮便宜约四成。

　　据介绍，纳武利尤单抗用法为每两周静脉注射纳武利尤单抗3mg/kg。也就是说，以50kg的患者为例，每个月需要注射300mg的纳武利尤单抗，即3大瓶，约需27780元。纳武利尤单抗的治疗具有“长尾效应”，一旦起效长期有效。
