Objective:Toinvestigatetheefficacyofpegylatedinterferon-α(PegIFNα)-basedtherapy,andestablish
strategiesforpatientswithhighHBsAglevelsatbaseline.
Methods:Theanalysiswasconductedinthedatafromamulti-center,prospectivereal-worldstudy
(OASISProject)fromChina,whichaimedatinvestigatingthebenefitsofanti-viraltherapies,and
startedrecruitingfromOctober,2020.Dataonthosewhohavecompleted48weeksoftreatment
beforeMay,2022wereanalyzed.
Results:Atotalof1508treatment-naivepatientsand1203treatedpatientswereincludedinthis
analysis.Intreatment-naïvepatientswithHBsAglevelslowerthan1500IU/mlatbaseline,20.3%
achievedHBsAglossafter48-weekinterferon(IFN)treatment.However,intreatment-naïvepatients
withwithHBsAglevelshigherthan1500IU/mlatbaseline,only1.3%achievedHBsAglossafter48-
weekIFNtreatment.NeitherIFNmonotherapynorIFNcombinationwithnucleosideanalogue(Nuc)
therapycouldimproveHBsAglossrateintreatment-naïvepatientswithHBsAglevelshigherthan
1500IU/mlatbaseline.
However,43.3%oftreatment-naïvepatientswithhighHBsAglevelsatbaselinecanhaveHBsAg
decliningandachieveHBsAg≤1500IU/mlafter48-weekIFNtreatment.Moreover,17.1%ofIFN-treated
patientswithHBsAg≤1500IU/mlatbaselineand24.5%ofIFN-treatedpatientswithHBsAg≤200IU/ml
atbaselinecanachieveHBsAglossafteranotherroundof48-weekIFNtreatment.
Conclusions:Treatment-naïvepatientswithhighHBsAglevelscanrarelyachievefunctionalcureafter
48-weekIFN-basedtherapy.However,highproportionsofthispopulationcanachievelowHBsAg
levelsafter48-weekIFNtreatment,andinIFN-treatedpatientswithlowHBsAglevelsatbaseline,
ahighproportionofthemcanachievefunctionalcureafteranotherroundof48-weekIFNtherapy.
ThissuggestsusprolongedorperiodicIFNtherapymightbeeffectivestrategiesforpatientswithhigh
HBsAglevelstoachievefunctionalcure.
